In order to to be tested at our one day Clinic you MUST have a family Doctor. Please ensure that you bring his or her information to the testing area.
YOU WILL NOT BE TESTED WITHOUT THIS INFORMATION. NO EXCEPTIONS
Each Individual agrees to indemnify and hold harmless Prostate Cancer Canada Network - Brampton (formerly US Too Brampton), Cruisin’ For a Cure Canada, Our testing Laboratories and The Powerade Centre including all of their Director's, Employees, Volunteers as well as their Officers, Agents and Servants against all liability for any loss or damages that the participant and/or family, relatives, or guests may sustain or incur as a result of claims, loss, costs, or judgments arising from the participants involvement in the Cruisin’ For A Cure Car Show. Participant(s)) acknowledge he/she/ they have liability insurance as required by the Province of Ontario Motor Vehicle Act and will not hold liable any of the above aforementioned.
